By filling out this Assistance Request Form you will be connected with Unite Us, an external vendor to the Division of Family & Children Services (DFCS). You will not be connected to a DFCS representative. Once this form is submitted, a Unite Us team member will contact you by phone within two business days to share more information about supportive resources local to you. Please make sure you include a frequently monitored phone number. There is no cost associated with this service, and the information you enter is completely confidential.
