Atlanta, GA – To protect the health and well-being of the public and staff, the Georgia Department of Human Services’ (DHS) Division of Family & Children Services is closing the Phase 2 Disaster SNAP (D-SNAP) location at Victory Baptist Church in Lowndes County (3229 Madison Hwy., Valdosta, GA 31601) due to a COVID outbreak, effective immediately. Today is the final day of Phase 2 D-SNAP operations.

The Victory Baptist Church site served residents of Berrien, Echols, Lanier, and Lowndes counties. Though the site is closed today, Nov. 4, 2024, eligible residents can still apply for D-SNAP through the program’s virtual option which includes:

  1. Submitting the pre-registration form; and 
  2. Completing a telephonic interview. 

Phase 2 applicants who are approved using the virtual option will receive their D-SNAP cards in the mail or at one of the other Phase 2 D-SNAP sites. Applicants who wish to apply in person can visit a neighboring D-SNAP site. The list of other Phase 2 sites can be found here

The pre-registration form can be found on gateway.ga.gov. Applicants will need to create an account to pre-register. Households may pre-register with just their name, address, and signature by clicking on the "Sign & Submit" button. However, DHS encourages applicants to answer as many questions as they can before submitting the pre-registration form. 

Households should be prepared to provide the following information: 

  • Name, date of birth, and Social Security number (if applicable) for each household member
  • Permanent residential address, temporary address, and/or mailing address
  • Proof of identification, such as a driver's license or state-issued ID
  • Amount of disaster-related expenses incurred during the disaster period
  • Amount of income for each household member
  • Amount of household resources, to include cash on hand or savings

D-SNAP applicants who live or work in the Phase 2 counties can pre-register by calling 833-DSNAP-GA (833-376-2742). Once an applicant has pre-registered, they can call the same number to complete their telephonic interview. The D-SNAP Virtual Call Center will be available to Phase 2 applicants until 5 p.m. today.
